The reporting of safety events, near misses, hazardous conditions and human errors is a necessary component for a good safety program. This session describes Speak Up, a program developed to provide safety professionals with information and tools that will help them create a just culture that will increase safety reporting. A just culture is one that is open, fair and based on trust when reviewing contributing factors, human errors and systems thinking, to a safety event before determining accountability.
