Attendees participate in a root cause analysis of oil/gas workplace accidents. The presenters lead a dissection of four real-life accidents from the oil and gas industry. These accidents help identify the contributing and root causes. Once identified, they discuss the required systems that must be in place to prevent similar accidents from occurring.
